With the Fi app installed, go into Settings Network & Internet Internet The gear next to Google Fi Access Point Names The 3 dots at the top Reset to default
This is the only fix I have found so far, but these steps will have to be performed every time you reboot or update Carrier Services.
Thank you for this fix!
The alternative, was uninstalling Google Fi every time I needed to shut down or reboot my phone, then reinstalling it and 'activating' after it boots... This is MUCH easier.

Currently im using DotOS which is based on android 11 .
Do i have to flash oos first ? Or its already on the zip file if i follow the clean flash procedure installation ?

Yes flash OOS first than rom than finalize(havoc finalize) than twrp and format data(yes). Reboot.
