据媒体报道，日本今年的首次轨道发射任务失败。
当地时间10月12日上午9点50分，日本Epsilon（艾普斯龙）运载火箭执行一项名为“Innovative Satellite Technology Demonstration 3（创新卫星技术演示3）”的任务。
尽管起初（前两级）一切正常，但网络直播画面显示， 第三级激活后似乎出现问题，发射人员启动了飞行终止程序，火箭遭解体摧毁。
日本宇宙航空研究开发机构（JAXA）随后表示，正在调查事故原因。
